目的：心不全患者を支援する看護師のACPの行動意図と行動への影響要因を明らかにする。
方法：慢性心不全看護認定看護師と心不全ケアを担う看護師822名を対象にした。計画的行動理論を元に作成した調査票で調査を行った。
結果：【ACPの行動意図】と【ACPの行動】には有意な相関があった。【ACPの行動意図】と【ACPの行動】には【ACPの主観的規範】【ACPの促進要因（介入スキル知識）】が強い影響要因であることと，【ACPの行動】のみに【ACPの抑制要因（障壁）】が負の影響要因であることが明らかとなった。認定看護師等の資格保有者と資格不保有者では【ACPの促進要因（介入スキル知識）】の差が大きく，資格保有者は資格不保有者よりも介入スキルを獲得していることが示された。
結論：心不全患者を担う看護師のACPの実践には【ACPの促進要因（介入スキル知識）】の向上と【ACPの主観的規範】を高める支援の必要性が示された。 |

Purpose: This study clarified nurses’ behavioral intentions for advance care planning (ACP) and factors influencing the behaviors of nurses who support patients with heart failure to help.
Methods: The study included 822 certified nurses’ providing care for chronic heart failure (certified nurses) and nurses responsible for heart failure care (uncertified nurses). A survey was conducted using a questionnaire developed based on the theory of planned behavior.
Results: There was a significant correlation between [Behavioral Intention of ACP] and [Behavior of ACP]. [Subjective Norms of ACP] and [Facilitators of ACP (Intervention Skills Knowledge)] were strong positive influences on both [Behavioral Intention of ACP] and [Behavior of ACP], while [Inhibitors of ACP (Barriers)] were negatively influenced only [Behavior of ACP]. The difference in [Facilitators of ACP (Intervention Skills Knowledge)] between the certified and uncertified nurses was significant, indicating that certified nurses had acquired more intervention skills than uncertified nurses.
Conclusions: Our results suggested that the nurses responsible for patients with heart failure need support to improve [Facilitators of ACP (Intervention Skill Knowledge)] and increase [Subjective Norms of ACP] in their practice. |
